They were effective and were necessary. It kept the use official. I was a dispatcher among other duties.

MB, I copied the forms from a PDF file, So I didn't actually have the original. I would guess the bottom one is 8-1/2" X 11" and that may help figure the smaller one.

I think I have one tucked away somewhere & as I recall it was on a light blue card-stock, probably about 3 X 5 or so. I'll see if I can dig it out later.

Another form to consider is QMC Form No. 237 - "Driver's Trip Ticket and Performamce Record" which was revised 1-1-42, and based on the fact that the about 500 copies I have were printed a few decades after that, the form definitely saw a continued use for many years.

For those WWII MV owners who like to do it by the book, there are a few forms that were part of the proper packing of a standard Field Desk which apply here.

Those forms are:
WD 39 - Investigating Officer's Report - Accident, Motor Transportation
Standard 26 - Driver's Report - Accident, Motor Transportation
Standard 27 - Investigating Officer's Report - Accident, Motor Transportation

But if it's a bad enough accident, you might need WD AGO Form No. 52 - Report of Death

In any case, make sure you're up to date on your VA Form No. 350 - Application for National Service Life Insurance

These forms do look at home in an uncluttered Jeep (or any other MV Glove Compartment).
